IMPORTANT please read instructions below before registering!

ATTENTION REGISTRATION FOR MAC USERS
If you are using an older version of Safari as your internet connection you may not receive a receipt or payment information when you submit your registration. We advice that you use Firefox or update your version of Safari on your MAC. We DO however receive your information through our admin.

If you do continue and have a problem, click here for a waiver and payment form. You will need to know the program fee to complete this form.

Email if you have any questions, achievelax@aol.com. All registers by submitting your registration you be automatically added to our mailing list.

PLAYER Registration Instructions: 

• Click on register here
• Click the program you are registering for
• Type all player information for each field
• Click SUBMIT
• You will receive a receipt/waiver
• Print out your receipt/waiver and have it signed by a parent if under 18
• Mail your receipt/waiver with payment to complete registration

Please do not print the registration form and complete by hand this is not an acceptable registration, thank you!

Achieve Lax Registration
All participating members must be fully registered, paid in full and waivers submitted prior or at their first game.  Once registration and payment is received you will be placed with your team or a team assigned to you.  We suggest that you register 2 – weeks prior to start date, sooner for sessions 2 and 3 ~ we fill very quickly!

TEAM  Registration for Leagues and Tournaments

• Team Coordinators are responsible for submitting a team roster online
• All Team Members are required to complete and submit a PLAYER registration online and mail or fax a signed waiver before playing.

Team members and free agents are not guaranteed to play until they are fully registered and payment has been received! 

Our Policy is, “First to Pay are First to Play!”

Free Agents Registration

For Leagues - Free Agents (individuals) will be assigned to a house team, or a team that does not carry the minimum of 11 players; and individual players fees will be required by all players. There is no guarantee that any individual will be placed on a team.

For Tournaments - We encourage only teams to register for Tournaments; however we will consider individual registration and do our best to accommodate those players.  There is no guarantee that any individual will be placed on a team. 

For Training Sessions and GEAR UP - We only take individual registration for these events.  However, if a Team would like to receive Team Training please contact Paula Evans, achievelax@aol.com and we will do our best to accommodate.
